Sally R Fiveash, 97 of Jacksonville, FL passed away Wednesday June 14, 2017
She was a life member of the Eastern Star, Rebeccas, & volunteered many years at the Salvation Army.
She is preceded in death by her husband Brooks F Fiveash.
Survivors include John H Fiveash, Debbie Hall, Carol Still, Cindy Redault all of Keystone Heights, many grandchildren and great grandchildren.
Gravesite inurnment service will be held at a later date at Evergreen Cemetery 4535 Main Street with Rev. Richard Petry officiating.
